I have a slightly different list that doesn't include Warzycha or Rosenthal. The list I have is the players who started the games so maybe they were subs. There's also one name on my list missing from yours.

Correct - Tony Dorigo was the last one, for Leeds.

I'm not sure of the exact percentages but I think it was reported that when Bobby Robson was England manager around 1990 he had over 80% of top flight players to choose from whereas now Fabio Capello has less than 30% of Premiership players eligible for England.
